Many thanks to the NBA staff i had know idea that i was entiled to any benifits but a friend showed me ad for them and suggested i call them. when i did they sent me the forms to take to the doctors and helped fill out all the paper work .about 3 months later they called me to say the application was suscessful and i would be receiving a cheque shortly ,my hat is off to the NBA and if you have a permmet injury i sugest you contact them ,if you are owed money they will get for you many thanks mike
